The Department of Education on Monday confirmed the death of a Grade 4 pupil following a routine deworming activity at Hugo Perez Elementary School-Annex in Cavite.

DepEd Cavite Province said it is coordinating with authorities as investigations into the incident continue.

According to the agency, the deworming activity was conducted inside the school on July 2 in coordination with the city health office. Several learners reportedly experienced discomfort afterward.

The Grade 4 pupil was hospitalized on July 3 after suffering from vomiting. Initial medical findings showed that the learner died from an infection that resulted in septic shock.

DepEd expressed its condolences to the pupil’s family, saying it was “profoundly heartbroken by the tragic passing” of the learner.

The agency said it is providing emotional, psychological, and logistical assistance to the bereaved family.

“We are likewise in constant coordination with the proper authorities concerned regarding the unfortunate incident,” the department said.

DepEd assured the public that the safety and well-being of learners remain its top priority and reiterated its commitment to protecting students’ health and welfare in public schools.

The department also pledged full cooperation with any investigation or legal proceedings arising from the incident.

“As such, should there be any ensuing legal processes and inquiries to be conducted by the duly constituted authorities in connection with the instant controversy, the DepEd Cavite Province officials, employees and personnel shall not shirk from our responsibilities and sworn duties, and will wholly cooperate and be amenable whenever the need arises,” the statement said.
